I understand that full payment due at time of registration. No refunds will be given for cancelations.

I understand that DeVote Performance Horses assumes no responsibility for injuries or illnesses which my child may sustain as a result of his/her physical condition or resulting from his/her participation in camp activities. In consideration of participating at camp, I hereby voluntarily release and discharge DeVote Performance Horses, its agents, contract services and employees from any and all claims for injury, illness, death, loss or damage which my child may suffer as a result of his/her participation in camp activities.

In the event of an emergency in which the parent/guardian cannot be contacted, the parents/guardian agrees to allow the staff members of DeVote Performance Horses to take appropriate action in the best interest of the child.

I understand that DeVote Performance Horses is not responsible for any personal items lost or stolen at our camp.

I permit DeVote Performance Horses to use picture of my child as a camp participant in promotional literature, promotional videos, DeVote Performance Horses website, and social media posts.

I understand that campers must wear pants and closed toed shoes for safety around horses. (riding boots preferred, but tennis shoes are acceptable) I understand campers must wear clothes that are acceptable to get wet due to water games and bring a towel and additional change of clothes to wear home. Shorts are acceptable for non-riding activities.

I understand campers are responsible to bring a sack lunch and a refillable water bottle.

I understand campers will be doing crafts daily. Children should be dressed in clothing that will not be ruined by tie dye, puff paint or other craft items.

I understand the above terms of camp and that this form confirms registration and there are no refunds for camps paid.
